Understanding DBT

The therapy for when feelings feel unmanageable

DBT was originally developed for people with borderline personality disorder, but its use has expanded far beyond that. It is now one of the most effective therapies for anyone whose emotions feel too intense to regulate, whose relationships keep blowing up, or who struggles with self-harm, chronic suicidal thoughts, or extreme mood shifts.

The core idea in DBT is dialectical: two things can be true at once. You can accept yourself exactly as you are, and you can change what is not working. DBT teaches concrete skills across four modules: mindfulness, distress tolerance, emotion regulation, and interpersonal effectiveness. These are not abstract concepts. They are tools you learn to use when you are about to do something you will regret.

At South Texas Mental Health Associates in San Antonio, we offer DBT-informed individual therapy with licensed clinicians. Sessions focus on the DBT skills that fit your situation, plus real-life practice between visits. Medication management and psychiatric care are available alongside if they help.

The 4 DBT modules

What DBT actually teaches

DBT is a skills-based therapy. Each module gives you concrete tools you learn, practice in session, and use when it matters.

1. Mindfulness The foundation of DBT. You learn to notice what is happening in your mind and body without reacting on autopilot, which is what makes every other skill possible.
2. Distress tolerance What to do when a feeling is unbearable and you cannot fix it in the moment. Skills like TIPP, ACCEPTS, and self-soothing help you get through without making things worse.
3. Emotion regulation How to identify emotions accurately, reduce vulnerability to intense reactions, and shift the intensity of emotions that are running the show.
4. Interpersonal effectiveness How to ask for what you need, say no, and manage conflict without damaging the relationship or your self-respect. DEAR MAN, GIVE, and FAST are the core frameworks.

No. DBT was originally developed for borderline personality disorder, but it is now used for a wide range of conditions where emotion regulation is a challenge, including PTSD, bipolar disorder, substance use, eating disorders, and severe anxiety.

STMHA offers DBT-informed individual therapy with a licensed clinician. Traditional comprehensive DBT programs include weekly group skills training in addition to individual therapy. If you need a full DBT program, your therapist can discuss whether STMHA's individual-focused model fits your needs or if a referral is appropriate.

It varies. DBT skills are typically taught over 6 to 12 months, sometimes longer. Some patients continue in maintenance therapy after that. Your therapist will discuss timeline after evaluation.

Yes. Many DBT patients also work with a psychiatric provider for medication management. At STMHA, both happen under one roof so care is coordinated.

Yes. DBT skills training and individual DBT work effectively by secure video. We offer telehealth across Texas.

DBT grew out of CBT. Both use behavior change and thought-pattern work. DBT adds acceptance, mindfulness, and specific skills modules, and was designed for higher-intensity emotional experiences.

We accept most major insurance plans for DBT-informed therapy. We verify benefits before your first visit.
